How the Program Works

In Sioux Falls:

1. Fill out online application below.
2. If eligibility requirements are met, a voucher will be emailed to parents/guardian to schedule an eye exam.
3. If the eye exam determines eyeglasses are needed, Sioux Falls Area Lions Clubs will pay the bill directly to the eye doctor.

Outside of Sioux Falls:

1. Fill out online application below.
2. If eligibility requirements are met, the application will be sent and reviewed by the Lions Club closest to you
3. They will notify you of approvals, arrangements or denials.
