How the Program Works

In Sioux Falls:

1. Fill out online application below.
2. If eligibility requirements are met, you will receive a vision voucher (sent by email, mailed to you, or available for pickup at the SD Lions Foundation, 3920 S Western Ave Ste 2, Sioux Falls, SD 57105). 
3. Take this voucher to The Eye Guys, 3825 S Western Ave, Sioux Falls, SD 57105. You do not need an appointment.
4. If the eye exam determines eyeglasses are needed, you can receive a pair of basic eyeglasses for $20.00. 

Outside of Sioux Falls:

1. Fill out online application below.
2. If eligibility requirements are met, the application will be sent and reviewed by the Lions Club closest to your address.
3. Your local Lions Club will review your information and contact you with your application status and any next steps.
